You will need Acrobat Reader (available for free) to view or print these forms in Portable Document Format (.pdf). PostScript files (.ps) may be printed by copying them to a PostScript-capable printer.

While we have attempted to give a brief answer as to required fees and locations to file the forms shown below, this information is subject to change, and you should read the specific form's instructions carefully. These forms are downloaded from the respective government agencies, and many forms have detailed instructions about how and where to file, which may vary with specific circumstances. Failure to file forms with the correct office or with the correct fee will result in your application or petition being returned to you and additional delays in processing. HELP! is available if you need it. Also, in some situations, the U.S. Citizenship and Immigration Services (USCIS) may waive fees.

Fee Waiver

Generally, the U.S. Citizenship and Immigration Services (USCIS) has discretion to waive any of its filing fees if the applicant can prove that he/she is unable to pay the fee.  The USCIS policy on when fees may be waived, and the process you must follow to request such a waiver may be found in the
USCIS Fee Waiver Policy Statement and the Fee Waiver Policy Memo of October 9, 1998 (.pdf file).

  • For assistance outside of the U.S., contact the nearest U.S. Consulate.

  • For inquiries on visa cases in progress overseas, contact the appropriate U.S. Embassy or Consulate handling the case.

  • For assistance in the U.S. with USCIS application questions, filing information, fees and forms, you may contact your nearest USCIS District Office or Sub Office. You may also call the national USCIS toll-free information service at 1-800-375-5283.

  • For assistance within the U.S. particularly regarding nonimmigrant visas, including Form OF-156, you may contact the State Department's Visa Office at 202-663-1225. You may also email a general inquiry to usvisa@state.gov. Be sure to indicate the general subject of your inquiry on the subject line (e.g., student visa, visitor visa, worker visa, etc.), and do not expect an immediate reply. You may also write to:
